Thankfully, she’s not far from the Myconid Colony in the Underdark, and if you have Medicine skills, you can save the life of one of the game’s most memorable NPCs.

Thulla is just one of the many NPCs you can help in the RPG game, and while her quest is a simple one in terms of gameplay, it’s an emotional journey that can be rewarding to complete. Here’s how to find the Antidote to cure Duergar Poison in Baldur’s Gate 3 in order to save Thulla.

Thulla can be found in the Myconid Colony in the Underdark. It’s important to enter peacefully and speak with her to learn that she’s been poisoned by the hostile Duergar.

There are a few ways to help Thulla, with the best being to find the Antidote. If you don’t have the required skills, there are alternative methods to discover, although they’re less effective. Speak with Thulla again and choose ‘How did you get injured?’ to pass a Medicine check.

If you fail this check, you can forcefully take Thulla’s boots, ending the quest. Alternatively, if you fail the check but have a high Intelligence score, you can discover “natural remedies” that don’t actually cure Thulla.

Cure Thulla in Baldur’s Gate 3

There are a few ways to cure Thulla, but the best two are to either find the Antidote or use magic to heal her. If you have a high enough Intelligence score and can persuade Thulla that she’s not in immediate danger, she can direct you to an Antidote.

If you don’t have any potions, you can talk to Sovereign Glut in the Myconid Colony to obtain one for a small fee. It’s also possible to obtain the Antidote without talking to Thulla first, but it’s much harder. To find the Antidote, you must venture to the Decrepit Village in the Underdark, which is infested with invisible Duergar Slavers.

The most straightforward option is to engage in combat with the slavers, with one of them holding the Antidote. You can recruit Sovereign Glut from the Myconid Colony to help in the fight, but this triggers another quest with its own consequences.

The strategy is simple: attack the slaver Gekh Coal with ranged attacks while another party member deals with Holvik. After defeating the slavers, you can loot their corpses and obtain the Antidote. Return the Antidote to Thulla to cure her and receive Speedy Boots as a reward.

If you’re a good-aligned character and want to ensure that the Decrepit Village is safe from the Duergar, you can inform Sovereign Spaw about clearing the Decrepit Village of the Duergar presence.
